How to Compare Physician Job and Side-Gig Offers: Pay, Hours and Terms

Use an offer worksheet instead of a single headline number. A meaningful comparison records the same fields for both roles and calls out any answer you do not yet have. Two rates can look similar while the work is completely different. Put scope, time and readiness next to the pay before choosing.

What the work involves

Use an offer worksheet instead of a single headline number. A meaningful comparison records the same fields for both roles and calls out any answer you do not yet have.

Who the work may fit

Record the patient or project scope, committed hours, preparation and administration, travel, coverage, pay terms and cancellation terms. Keep unknowns visible. Do not fill them with what another employer usually offers.

Questions before you say yes

  • What work and time am I actually committing?
  • Which part of compensation is guaranteed, and what is conditional?
  • What expenses or unpaid tasks remain mine?
  • What must clear before I can begin?
  • What costs or obligations remain when I leave?

Your first step

Build a two-column worksheet with one row for each question. Ask the hiring teams to fill the gaps in writing before you decide.

What can go wrong

Calculating a take-home estimate without knowing expenses, taxes or actual paid hours. This worksheet compares terms; it does not forecast income.

How to get started, step by step

Turn the idea into a specific next action. These are preparation steps, not approval to perform a role.

  1. Write one row for scope, schedule, paid tasks, expenses, coverage, readiness and exit terms. Leave blanks visible.
  2. Record both offers in their original pay units. Do not mix per-visit, hourly, per-shift or project prices into a supposed range.
  3. Ask each hiring team for the missing terms in writing, including which pay is guaranteed and which is conditional.
  4. Compare the complete commitment and review unclear contract or tax terms with an adviser before choosing. This worksheet does not forecast take-home income.

Where to look and who to ask

Use search terms and contacts that fit this work. Check the actual source and current requirements; these suggestions are not verified openings or Saile inventory.

  • Go back to the actual written offer and the hiring team for answers; another employer’s habits cannot fill a blank.
  • Use the IRS classification resource for federal employment-tax factors and a professional adviser for your actual arrangement.
  • Use the relevant role guide to identify clinical readiness or project-deliverable questions before treating the bigger headline as the better offer.

Scheduling and payment realities

Put preparation, travel, orientation, documentation, availability windows and revision rounds next to paid work. Ask what remains owed or required when the role ends. A larger nominal rate can represent a different commitment rather than a better like-for-like offer.

Keep gross pay and taxes separate

For federal employment taxes, the IRS considers control and the relationship, not only the contract label. Self-employed individuals generally pay income and self-employment tax and may need estimated payments. Keep payment and expense records and ask a tax professional about your actual arrangement; this guide does not calculate take-home pay.

Before you say yes: a final checklist

Do not let an unanswered question become an assumed term.

  • Scope fits my expertise and any required authorizations.
  • Time, deliverables, payment unit and expenses are written.
  • Outside-work approval, conflicts and confidential-data handling are checked.
  • Coverage, escalation or review responsibilities are clear.
  • Starting and ending the engagement each have a written process.
